The da Vinci is the first of a number of revolutionary systems that can assist the performance of laparoscopic surgery by allowing the surgeon to operate remotely from the operating table using articulated instruments that mimic the motions of the hands and wrists. Although there is no component of the system that is currently autonomous, and therefore it is not "robotic", the system does translate of normal psychomotor functions to the peritoneal cavity and also provides stable stereoscopic video visualization of the operative field, all under the remote control of the surgeon. As with all laparoscopic surgery, ports must be appropriately placed, uterine manipulators, where needed, positioned optimally and continuous care must be taken to use energy-based and other instrumentation in a fashion that minimizes the risks of injury to vital structures. At this time, there is no evidence that da Vinci or any other similar system improve clinical or cost outcomes with laparoscopic gynecologic surgical procedures but it is likely that future developments will ultimately prove beneficial for patients and surgeons.
