HYSTEROSCOPIC SURGERY

Uterine  Local Anesthesia for Hysteroscopy

Virtually any hysteroscopic surgical procedure can be performed under local anesthesia in an appropriately equipped office procedure. This is the local anesthetic technique reported by Dr Munro in 2014. Not included in this video is demonstration of the "fundal block" that may well be necessary for transactional procedures high in the fundus where T10 typically contributes innervation.

Type 2 Hysteroscopic Myomectomy:

Performed In Office Under Local Anesthesia

Background for this video is also provided in the section on Hysteroscopic Surgery. This is a FIGO Type 2 leiomyoma. The entire procedure was performed under local anesthesia. You will see how entering the pseudo capsule results in partial "release" of the leiomyoma that is further enhanced by sharp and blunt dissection with scissors. An electromechanical morcellator is used to remove the tumor. The patient drove herself home immediately following the completion of the procedure.

LAPAROSCOPIC VAGINAL CUFF CLOSURE

Three Port Technique - Midline - Ipsilateral - Laparoscopic Vaginal Cuff Closure

This video created by Dr Andre Petrikovits shows a three-port technique for vaginal cuff closure using a midline suprapubic and ipsilateral lower quadrant technique.

LAPAROSCOPIC TISSUE EXTRACTION / MORCELLATION

Minilaparotomic Morcellation Technique

Video by Dr Saad and presented at ACOG in 2016 demonstrating manual morcellation with a scalpel as well as a simple but effective simulation system
UNIQUE LAPAROSCOPIC TECHNIQUES
Transumbilical Endometrial Biopsy
Dr Rohan Hattiangadi and Dr Seth Kivnick describe a mechanism for performing intraoperative transumbilical endometrial biopsy prior to performing laparoscopic hysterectomy. This video was presented at the AAGL Global Congress of Minimally Invasive Gynecologic Surgery in Las Vegas Nevada in November 2018.
